Objective: Determine the efficacy and safety management with adrenaline versus albuterol nebulized in pediatric patients diagnosed with acute bronchiolitis in Hospital Niño Jesus and Barranquilla district hospitals, during Janu- ary 2012 to April 2013. Materials and Methods: Controlled clinical trial in patients with acute bronchiolitis; nonrandom convenience sample (50 patients per group). Patients were treated with a scheme of 3 sprays every 20 minutes with 1 % adrenaline (0.5 mg / kg adrenaline group) or 0.5 % salbutamol (0.15 mg / kg, salbutamol group). Before and after the application of therapeutic scheme evaluation was performed. Evaluations were done at 30 and 60 minutes, applying scale Tal. Results: Upon admission, patients receiving adrenaline were classified as moderate and severe 88 % 12 %; after 60 minutes were classified as mild 48 %, moderate 50 %, severe 2 % (control 60 minutes OR = 0.67, CI = 0.56 to 0.78, p <0.00001); patients receiving scheme with salbutamol, were classified as moderate income 92 % and 8 % severe, at 60 minutes were classified as mild 22 %, moderate 74 % and severe 4 % (control 60 minutes OR = 0.57, CI = 0.47 to 0.67, p = 0.0003). Conclusions: Nebulized adrenaline is more effective than nebulized salbutamol in the management of acute bron- chiolitis in population under 1 year old.
Objetivo: Determinar la eficacia y seguridad del manejo con Adrenalina versus salbutamol nebulizados, en po- blación pediátrica con diagnóstico de bronquiolitis aguda, en el Hospital Niño Jesús y hospitales del distrito de Barranquilla durante enero de 2012 a abril de 2013. Materiales y Métodos: Ensayo clínico controlado, en pacientes con bronquiolitis aguda; muestra no probabilística por conveniencia (50 pacientes por grupo). Los pacientes se trataron con un esquema de tres nebulizaciones cada 20 minutos con adrenalina al 1 % (0,5 mg/kg, grupo adrenalina), o salbutamol al 0,5 % (0,15 mg/kg, grupo salbu- tamol). Se realizó valoración previa y posterior a la aplicación de esquema terapéutico, se hicieron evaluaciones los 30 y 60 minutos, aplicando escala de Tal. Resultados: Al ingresar los pacientes que recibieron adrenalina se clasificaron como moderados 88 % y severos el 12 %; a los 60 minutos se clasificaron como leves el 48 %, moderados el 50 % y severos el 2 % (Control 60 minutos: OR= 0,67; IC= 0,56 - 0,78; p < 0,00001); los pacientes que recibieron esquema con salbutamol se clasificaron al ingreso como moderados el 92 % y severos el 8 %; a los 60 minutos se clasificaron como leves el 22 %, moderados el 74 % y severos el 4 % (Control 60 minutos: OR= 0,57; IC= 0,47 - 0,67; p= 0,0003). Conclusiones: La adrenalina nebulizada es más eficaz que el salbutamol nebulizado, en el manejo de la bronquio- litis aguda en población menor de un año.
